Community Health Worker
Community health workers, also known as health navigators or lay health workers, are trained professionals who work directly with individuals and communities to promote health and well-being. You may benefit from having a CHW if you need assistance with:
- Access to Healthcare
- Health Education and Promotion
- Chronic Disease Management
- Cultural and Linguistic Support
- Advocacy and Empowerment
Community Supports
- Up to $5K in security deposit assistance
- Assistance finding and sustaining home
- Assistance with landlord coordination
Community Health Worker Training
- 10-Day certified training
- In-person or virtually
- Additional continued education for CHW’s (Violence Prevention Program, Gang Intervention Program, Asthma Self-Management)
